​Wellness Checks:  Designed to provide a peace of mind and reassurance that all is good with your loved one. 

A notification will be sent after each visit with check in notes provided.

Safe and Steady Program and Service: Designed to prioritize safety and well- being with a certified   

Age Safe® America Senior Home Safety Specialist. This service includes a home safety assessment and fall         prevention plan for potential hazards and recommendations. This can be scheduled as one-time visits, annually, quarterly, and biannually.

Post Hospital Home Support Transition Program: Designed for a short-term solution after hospital discharge

to help ensure a smooth transition back home for up to a month after discharge. 

Pairs greatly with the Safe and Steady service.

Memory Moments: Designed as a dedicated dementia support program to enhance quality of life. Structured routines ​and activities that incorporate memory-friendly activities and brain games with others.
